Direct sequencing is a service only available to one group in the CRUK-CI institute that operate their own sequencer. Do not use this form in any other circumstance.
Assuming you have access to direct sequencing, use this form only if your submission meets both of these criteria:
Do not use this form if you wish to sequence on another sequencer or if you would like Genomics to take care of the preparation and sequencing. In these cases, use the "sequencing only" form above.
